Wegovy vs Zepbound: what self-pay really costs in 2026

If you're paying out of pocket, the price you pay for Wegovy or Zepbound depends mostly on three things: which strength you've been prescribed, whether you're a new customer, and whether you buy straight from the maker. Here's what Novo Nordisk (Wegovy) and Eli Lilly (Zepbound) publish as their own self-pay prices right now.

Both programs need a valid prescription for an FDA-approved use. Neither counts toward your insurance deductible. This page covers brand-name products only.

    How Wegovy self-pay works

Novo Nordisk sells the Wegovy pen through NovoCare Pharmacy at a flat $349 a month for every strength from 0.25 mg to 2.4 mg, and $399 for the higher-strength Wegovy HD 7.2 mg. People new to the offer pay $199 a month for their first two fills of the 0.25 mg or 0.5 mg pen, through December 31, 2026. After that, the price goes back to $349.

Novo also sells a Wegovy pill for self-pay customers. Its price ranges from $149 to $299 a month depending on strength ($149 for 1.5 mg, $199 for 4 mg, $299 for 9 mg and 25 mg). It's a different product from the weekly pen, so it isn't in the table above.

    How Zepbound self-pay works

Lilly sells Zepbound vials and the multi-dose KwikPen through LillyDirect. The 2.5 mg starting strength is $299 a month (2.5 mg is a starting dose, not a maintenance dose, per Lilly) and 5 mg is $399. The higher strengths are $449 a month under Lilly's "Self Pay Journey Program," but only if each refill is bought within 45 days of the last delivery. Miss that window and the price rises to $499 for 7.5 mg or $699 for 10 mg and up.

Lilly lowered these vial prices on December 1, 2025. Before that, 2.5 mg was $349 and the other strengths were $499.

    What to watch for

    • Intro offers end. Wegovy's $199 price lasts for two fills only, and the offer ends December 31, 2026.
    • Refill timing matters for Zepbound. A late refill can add $50 to $250 a month.
    • Supplies cost extra for Zepbound. Budget for needles or syringes.
    • Prices can change. Both companies say they can change or cancel these programs at any time.

    FAQ

Which is cheaper to pay for myself?
It depends on the strength you've been prescribed and how long you've been buying. At the published prices, Wegovy's standard price is $349 a month and Zepbound runs $299 to $449 a month. Your prescriber decides what's right for you.
Can I use my insurance and also get these prices?
These are cash prices. Money you spend doesn’t count toward your deductible or out-of-pocket maximum. If you have coverage, both makers list separate savings offers for insured patients.
Are these the same as compounded versions?
No. These prices are for FDA-approved, brand-name Wegovy and Zepbound sold by their makers. Compounded drugs are not FDA-approved, and this page doesn't cover them.
    Do I need a prescription?
Yes. Both programs require a valid prescription for an FDA-approved use.
